These four documents are required for the registration process and the try-out process.

No player will be allowed to play or try-out without the necessary forms.

The forms that need to be turned in (or completed during the online registration process) are the following:

• Parent Code of Conduct
• Consent to Treat Form and Medical history form
• Waiver of Liability

Print these forms out and bring them with you to try-outs if you haven't completed them during the registration process.
